A 54 year old male C/O abdominal bloating and sob since 2 -3 months.C/o pain in left side of chest - h/o trauma to chest yesterday

C/o pain in left side of chest - h/o trauma to chest yesterday
Hopi :
Patient was apparently asymptomatic 2 to 3 months ago then he had developed abdominal bloating, which aggravated after food intake, which relieved after taking PPIs , belching present, no regurgitation
H/o sob on exertion ( grade l mmrc ) which aggravated since 3 months after bloating of abdomen
No C/O chest pain ,palpitations,orthopnea,pmd, decreased urine out put/facial puffiness
H/o trauma to left 8th rib yesterday while doing routine labour work , and continuous pain in the left side of chest, dragging type pain , aggravates on movement and during deep inspiration
N/k/c/o fever cold cough, vomiting, loose stools
Past history:
N/K/C/O HTn,DM,CAD,CVA,TB , asthma and epilepsy
